What is an AT Evaluation?
It’s used to determine if there are any interventions that may help an individual in school and/or their day to day life.
An AT Evaluation or Assessment is usually done after a need has been determined due to things such as:
Visual or Auditory Impairment
Intellectual/Developmental Disability - Cerebral Palsy, Autism
Learning Disability - Dyslexia, Executive Function
In schools this can be requested by teachers, the I&RS team, the IEP, another evaluator/service provider, or the parents of the child.
What information is needed to complete an AT Evaluation?
For an AT evaluation to take place, the individuals who work with the student on a regular basis are requested to complete Pre-Eval Questionnaires. On top of that, the student's IEP (if they have one) and any other evaluations or assessments the student has previously had are also requested as a part of the pre-eval process. These could include evaluations from the PT or OT, a Functional Vision Assessment, the Psychological Evaluation, just to name a few.

The Paren/Guardian Questionnaire is REQUIRED.

The Special Education Teacher Questionnaire is REQUIRED, if the student does not have a dedicated Special Education Teacher, then the General Education Teacher Questionnaire can take its place.
